How to Get Medical Marijuana Identification Card?

  • Get a Physician Written Certification Form from your physician. It must be filled and signed by your physician within 90 days before you submit the application form to the department for a medical marijuana card.
  • You have to complete the application and put your signature on it. You can even sign up for a caregiver in case you need assistance with the medicine.
  • Pay application fees. The fee depends on the plan you choose between one-year plan, two-year plan or three-year plan.
  • A proper passport size photograph must be attached along with the application form. Go to a local studio to get a 2×2 inch photograph. Make sure the picture captures your face perfectly and your face is clearly visible.
  • You have to submit two proof of residency. You have to prove that you are an Illinois resident. You can give anything from the list – state ID, bank statement, driver’s license, voter ID, or utility bills.
  • You should also produce proof of age and identity. So you can simply provide a copy of driver’s license or a copy of the passport.
  • You must also provide your fingerprints for further security. You have to first sign a fingerprint consent form.
  • If you want to elect a caregiver, a separate form must be filled for the caregiver with the same information, that is, photograph, identity proof, address proof, and signature of the caregiver.

Once you get your medical marijuana identification card, you can go to any licensed dispensary to get medical marijuana. If you have opted for caregiver, a caregiver must collect medical marijuana for you.

You can select the licensed dispensary from where you wish to buy medical marijuana. You can’t just buy from any other dispensary that is not licensed to sell medical marijuana. You can change the dispensary if you wish to without any fees.

The Illinois Department of Financial and Professional Regulation has licensed about 55 dispensaries to sell medical marijuana.
